A patient presented with acute otitis externa and spreading facial cellulitis. On admission, he was started on intravenous cloxacillin and ofloxacin ear drops. While his general condition continued to improve, the systemic physical examination and his abnormal routine blood parameters suggested an underlying hematologic malignancy. To the best of our knowledge, this is the first report in the English literature of acute otitis externa with facial cellulitis as a masquerading feature of chronic lymphocytic leukemia.
